At Saint Moscati we are all part of a community that cares and looks after one another and as our patient you and your health are at the center of our attention. The Saint Moscati team will gladly help you in making the transition to Saint Moscati as smooth and easy as possible. 

• The first step is to call us at (971) 273-0679 to set up an appointment.
• You can then register online by clicking here and creating a patient portal. 
• After your appointment is set up you can fill out the new patient forms below or come in on the day of your appointment and fill the forms out then.  
• Once filled out you can print them and bring them in.
